Information for New Patients - How to make your first visit a success: Plan to arrive a few minutes early to complete the necessary registration and insurance information. Bring a valid Picture ID like Driver's License. Please arrange to have your medical records and any diagnostic images (MRI, CT Scan, XRays) sent in advance or bring a copy with you. The physicians like to see the actual scan, not just the report. These records may be pertinent to our evaluation of your neurological system. We will be able to better serve you with the most information available in hand. Patients should contact their primary care physician or referring physician to have records faxed to the office at 972-306-6500. You may need to sign a release to have the records transferred. Be sure to include the medication name, prescribed dosage, prescribing physician's name, and how often you take each medication. We will also need information about your insurance coverage. If any of this information changes between visits, please let us know so we can update your records. If your insurance company requires a referral from the primary care physician, you must have it faxed or bring it with you. Insurance co-payment is due at time of service. We do accept Visa, MasterCard, American Express, check or cash. We accept most insurance companies but we ask that patient's check with their companies to make sure we are a participating provider. If you are covered by an insurance company that requires pre-certification and authorizations for procedures (e.g: Sleep study, EEG, EMG), please inform our office so that we can assist you. Prescription Refills: Please note that we require at least 24 hours to complete a prescription refill request. Please have your pharmacy fax your refill request to 972-306-6500 or via e-script.
